- Full-time employees who average 30+ hours a week and earn at least $26,000 a year
- Part-time employees (30 hours a week or under, earning at least $16,000 a year) may also be eligible
- W-2 employees only. 1099 contractors aren't eligible.
- Employees currently enrolled in Medicaid aren't eligible
